Permohonan informasi secara lisan kepada petugas :
Pemohon mendatangi petugas di bagian Informasi. Petugas menanyakan hal informasi yang dibutuhkan oleh pemohon. Petugas memberikan informasi kepada pemohon. Petugas menanyakan kepada pemohon apakah pemohon sudah memahami informasi yang diberikan. Petugas menjelaskan informasi yang belum dipahami oleh pasien. Petugas tidak mengulangi menjelaskan informasi apabila informasi sudah dipahami oleh pasien. Petugas memastikan pemahaman pemohon dengan metode umpan balik.

Pemohon menghubungi bagian Informasi melalui telepon Telepon dari pemohon yang berisi permohonan informasi yang diberikan melalui bagian informasi wajib dan harus ditanggapi.
